Workers exposure survey
According to articles R231-54-5 and R231-56-10 of the french Code du Travail, every company must provide to its employees a sheet where all the chemicals they were exposed to all along their career is detailed. This sheet can be used as a basis to a acknowledgment of professional diseases dossier.
It is forwarded to the work physician and shall be stored for 50 tears after the end of the exposure.
What is a dangerous chemicals?
This is a product inducing a health hazard. It is classified as: very toxic (T+), toxic (T), Harmful (Xn), Irrritant (Xi), Corrosive (C), Sensibilizing or Carcinogen, Mutagen or Reprotoxic.

The exposure sheet contains the following information:
- Employee's IDs: First name, Name, Birth date
- Function
- Exposure period
- Tasks
- Product caracteristics: classification, exposure limits
- Exposure controls: date and results
- Preventive measures: Protective equipments
- Other hazards at the workplace: noise, vibrations...
- accidental exposure: date, length and gravity.
